- I will require the payment of *£280 to secure your booking. You will be sent a link to pay electronically on confirmation of your booking.
- The final balance of *£300 is payable before I can email you the written report.
- Information provided within completed questionnaires by parents and school will be used to form part of the background information within the report.
Cancellation policy
If you need to cancel your assessment, you will need to notify me by email at amanda@successwithdyslexia.co.uk
Cancellation charges will be applied as follows:
- Cancellation within 14 days of a booked session will incur a loss of the *£280 booking fee.
- If an assessment is cancelled between the period of booking and any time outside of 14 days from the assessment date, I will refund you *£180 of the £280 deposit you paid.
- In the unlikely event that I need to cancel due to unforeseen circumstances, I will rearrange another mutually convenient date, or arrange a full refund.
- If you require to change a booking date, there will be no charge outside of 14 days of the assessment date. Within 14 days of an assessment date a charge of £150 will be incurred and added to the final invoice amount.

Report Preparation
Following the assessment, please allow up to 21 days for the report to be prepared and emailed to you upon receipt of the final balance.
Requirements
- A valid eye/hearing test conducted within the past 12 months is a pre-requisite before assessment can take place.
- Completion and receipt of the parents' questionnaire and school questionnaire, which include the visual difficulties screening checklist, are mandatory prior to the assessment.
